FAQs About Cosmetic Surgery in Santa Barbara

During cosmetic surgery consultations at our Santa Barbara Cosmetic Surgery Center, our patients ask many questions – and not just about the procedure! To help you make a more informed choice about cosmetic surgery, here are some of our most popular questions that are asked, and our answers.

Q: I am not sure if I am ready for surgery, but I am interested. Should I come in now for a consultation?
A: We recommend coming in for a consultation when you have specific concerns. We will evaluate your concerns and together come up with a plan.

Q: How long do I have to wait from consultation to the day of surgery?
A: Generally your surgery can be scheduled within 2 to 4 weeks after your consultation.

Q: Where are the surgeries performed?
A: We perform all of our surgeries in our outpatient facility that is located near Cottage Hospital.

Q: Do you take medical insurance? Will you bill my insurance company?
A: No. We do not take medical insurance and we will not bill your insurance company.

Q: How much will my cosmetic surgery cost?
A: Cost depends on the procedure(s) performed. A written quotation will be given to you at the time of your consultation.

Q: Do you have payment plans?
A: Yes, you can set up a payment plan through Care Credit. Read more about Care Credit here.

Q: Is my medical information confidential?
A: Your medical information is always kept private. It can be discussed with your family member only if you say OK.

Q: Do I have to go under general anesthesia?
A: No. All of our facial surgeries and body liposuction can be performed safely and comfortably using local anesthesia and conscious sedation.

Q: How much pain can I expect after surgery?
A: Most facial procedures (including full face lifts) and body contouring surgeries have remarkably little pain – and can be easily controlled with oral medication.

Q: I have some medical issues – can I still have surgery?
A: It will depend on what your medical issues are. As cosmetic surgeries are usually elective, there is never a reason to put any patient at risk. If necessary, “medical clearance” will be requested from your family MD.

Q: I’m from out of town. Where do you recommend that I stay?
A: We have a webpage for visitors to Santa Barbara including hotel recommendations, dining options, and attractions that you may be interested in during your visit.

Q: I live locally, can I drive myself home?
A: No. Please make arrangements for a friend or relative to take you home after the surgery.

Anti-Aging Cosmetic Surgery Options in Santa Barbara

The aging process happens every day and nothing can stop it but with the help of cosmetic procedures we can turn back the clock on your appearance a few years or more.

For Moderate Wrinkles and Sagging
Face Lift surgery is the best option for those who have moderate wrinkles and sagging skin on the face and neck. This surgery will reposition, redefine, tighten skin, and support deep tissues with the end result looking natural and rejuvenated.

For Mild Wrinkles
Some people may be experiencing signs of aging but are not quite ready for a full face lift. At our Santa Barbara Cosmetic Surgery Center, we offer minimally-invasive surgery options for the forehead, cheeks, eyes, neck, and hands that will give noticeable improvement with less cost and lower downtime.

For Early Signs of Aging
If you’re not ready for surgery, there are medical spa treatment options. Dr. Perkins is the founder and owner of Evolutions Medical in Spa Santa Barbara where clients can have non-invasive skin tightening treatments done including Total Tightening and Ultherapy. Both are skin tightening and collagen-boosting treatments that result in a firmer, “fresher” look.

Some of our patients who are good candidates for minimally-invasive (MI) cosmetic surgeries that have less downtime, less cost and give noticeable improvement. This is ideal for those who are experiencing early changes and would like to delay a larger surgery or for those who want to “maintain” the benefit of a previous surgery. Minimally Invasive Cosmetic Surgery in Your 40s

Cosmetic surgery is becoming more prevalent among 40-somethings than ever before. The upward trend amongst this age group may stem from our culture’s obsession of staying “forever young” coupled with the pressure to look good in social media pictures. But most importantly, the 40-somethings that we see at our Santa Barbara Cosmetic Surgery Center simply say that they don’t “feel in their 40s” even though their face may say otherwise.

Aging is always happening, but the decade of the 40s is usually the time when visible signs start to appear. Collagen and elastin production in the skin slows down while sun and environmental damage start to show up, and the result is fine lines, wrinkles, age spots, skin laxity and sagging.

While there are many “non-invasive” options such as Botox / Dysport, dermal fillers, and lasers, more 40-somethings are turning toward more permanent options such as cosmetic surgery because they aren’t interested in multiple med spa visits to keep up with Botox or fillers and/or they are no longer getting the optimum results from those treatments.

The good news is that for most people in their 40th decade, there are more options than if they waited two more decades to rewind the clock. The essential foundation of the face is still present in the 40s and often minimally invasive (MI) cosmetic surgery procedures can be used to achieve the desired result. (We must add that MI procedures can be a good option for those who want to “maintain” the benefits of a previous plastic surgery.)

MI cosmetic procedures give noticeable improvement, cost less, and have less recovery “downtime”. In addition, they have smaller or no visible incisions. MI treatments can address all areas of the face – the forehead, eyebrows and temporal region, lower eyelids, cheeks and mid-face, and the area under the chin (submentum) – and the hands.

Cosmetic Procedures and Plastic Surgery Trends in 2019

Cosmetic procedures and plastic surgery will continue to rise in 2019, however, the days of getting over-exaggerated plastic surgery (overfilled lips, disproportionate breasts) are on their way out. The future of plastic surgery and non-invasive cosmetic procedures (i.e. dermal fillers, Botox, lasers) is trending towards enhancing what a person already has and making slight tweaks so they look more refined “naturally” while still staying true to their inherited features.

Here is what we expect to see in 2019 on the cosmetic and plastic surgery front.

Minimally-Invasive Treatments for Preventative Measures or Refinement

More people will seek Minimally-Invasive (MI) cosmetic surgery including those who want to delay a larger plastic surgery or those who want to “maintain” the benefit of a previous surgery. MI surgery is ideal because it can give a noticeable improvement with less cost and less “downtime”.  In fact, one MI procedure can often be much less of an expense than multiple medical spa treatments. MI surgery can treat many facial areas, the area under the chin, and hand/body contour defects. Here is a brief description of MI procedures that we do:

  • A Temporal Lift elevates the outer half of the brow and improves crow’s feet.
  • An MI Cheek Lift tightens tissue with a small amount of skin excision to replace lost volume to the cheek area. Volume can also be added to the mid-face, nasolabial folds, and pre-jowel area through injections of your own fat (taken from a place where you want to have less of a fat bulge).
  • Lower Eyelids can be treated by removing a small “pinch” of excess skin with fat removal from inside the lower lid (so there are no sutures or external scars).
  • Add Facial Volume via fat grafting. Fat can be places in the brows, temporal hollows, under the frown lines, deep circles around the eyes, apples of the cheeks, cheekbone region, groove between the nose and the corner of the mouth, lips, pre-jowel area (marionette lines), and the jawline.
  • Hand Rejuvenation can be done by fat transplantation to replace volume loss in the back of the hands. Additionally pigment spots can be treated with lasers.
  • A Submentoplasty is less extensive than a neck lift and is especially good for men who are starting to see a “turkey neck”.

Injectable Cosmetic Procedures to Stay on the Rise

Injectables (Botox, Dysport, or dermal fillers like Restylane) will stay on the rise because of the accessibility and the affordability of these treatments. They are also appealing because most results are instantaneous with little to no downtime. A dermal filler procedure can easily be done on an extended lunch hour. We are also seeing a younger audience turn to injectables for enhancements to help them look their best in their close-up “selfie” photos they post on social media.

Body Contouring Will Continue to Be Popular

There are many non-invasive body contouring procedures that have been introduced over the past few years including freezing fat or melting it off for fat reduction in a targeted area. People are drawn to these treatments because they do not involve surgery and have less of a downtime for recovery. Most of these treatments claim up to 20% reduction – this is optimal and may take more than 1 treatment to achieve. They may improve contours but results may be minimal and not noticeable to other people. Multiple treatments may actually cost more than one liposuction treatment for the same area. At the Cosmetic Surgery Center, we offer liposuction to improve body contours and proportion. Liposuction results are guaranteed and skin excision can take place at the same time, if necessary. People no longer need to go under general anesthesia for liposuction as it can be done comfortably using a combination of local anesthesia, oral and intravenous sedation. If you are looking for a dramatic result in one treatment, liposuction is a good choice.

Liposuction Body Contouring in Santa Barbara

Do you have a fat bulge on your body that is resistant to diet and exercise?

If so, you are not alone. Many women and men have problem areas that they cannot reduce and that is when they look to a cosmetic surgeon for help.

Lately we have seen ads for very enticing non-invasive body contouring options, however liposuction remains the most reliable and quickest way to significantly improve contour issues on any part of the body. Liposuction is a “one and done” treatment and if there is excess skin, it can be removed at the same time.

At Cosmetic Surgery Center in Santa Barbara, we offer liposuction for many parts of the body:

  • Arms: The most common concern is the upper arm area (above the elbow) where fat and excess skin can result in a “bat wing” appearance. Fat can be removed through liposuction and if skin removal is needed, the scar is placed on the inside of the arm (against the body) so it is not visible from the front or back.
  • Abdomen: Bulges in the abdomen area can be treated with liposuction. Incisions are hidden in the navel, pubic hair, or under the bikini line. If skin removal is needed, often a “mini tummy tuck” is sufficient.
  • Legs and Thighs: Inner and outer (saddlebag area) thighs can almost always be treated with liposuction. The overall goal is to decrease bulk with a smooth end result.
  • Knees, Calves, and Ankles: These areas should only be done by experienced surgeons as fat removal needs to be precise and conservative while leaving a normal appearing shape that blends naturally into surrounding contours. Over-treatment can be difficult to correct.

Many people are surprised to find out the following facts about liposuction when done in our Santa Barbara cosmetic surgery practice:

  • General anesthesia is not needed; Oral and IV sedation plus dilute local anesthesia allows us to comfortably perform the procedure.
  • Incisions are small (less than 1/4 inch) and are placed in “hidden” locations.
  • A blunt cannula is used to selectively remove fat without injuring other tissues.
  • Post-operative pain is usually minor (and controlled with Tylenol or ibuprofen).
  • Light physical activity can be resumed a few days after treatment.

Is liposuction the right choice for you?

If you have a larger amount of fat, are looking for a dramatic change, or just want to do 1 treatment and be done, then liposuction is a great choice.

Minimally Invasive Cosmetic Surgery Options in Santa Barbara

Do you want to improve your appearance with lower cost and less downtime?

Minimally invasive (MI) surgery is intended to give you noticeable improvement with less cost, less “downtime”, as well as smaller or no visible incisions.

Ideal candidates for MI surgery include:

  • People with early changes
  • People who want to “maintain” the benefit of a previous surgery
  • Medical spa clients who want to delay a larger surgery

Our Santa Barbara cosmetic surgery practice offers a variety of MI surgery procedures:

Temporal (Brow) Lift: Open up the eye area so you look more approachable (instead of tired, unhappy, or angry) through a brow lift that elevates the outer half of the brows and improves crow’s feet. A 1/4″ incision is hidden in the hair.

MI Lower Eyelid Surgery: Improve the lower eye area for a more youthful, rested appearance by removing a small “pinch” of extra skin that will tighten lower lid wrinkles or by removing fat from inside the lower lid (leaving no visible scar).

MI Cheek Lift: Lift and restore the cheek area (with the option of adding volume) by tightening the deep supporting tissue with a small amount of skin removal. During the procedure, you may choose to add volume to the mid-face, nasolabial folds, and/or pre-jowel area through injections of your own fat to further enhance your results.

Fat Transplantation (or Fat Grafting): Improve age-related volume loss almost anywhere in the face or body though fat grafting. A small of fat is taken from an unwanted bulge using a small cannula and the fat is redistributed.

  • Fat Transplantation for the Face: Restore youthful contours. Fat can be place in the brows, temporal hollows, under frown lines, deep circles around the eyes, apples of the cheeks, high cheekbone region, nasolabial folds (groove between the nose and corner of the mouth), lips, pre-jowel area (marinate lines), and the angle of the jaw.
  • Fat Transplantation for Hands: Fat is used to fill the back of hands to obscure prominent veins and tendons that show as we age. Lasers or peels can be done to address brown (“liver”) spots for an even more desirable result.

MI Neck (Submentum): This is a good option for those who are starting to see a “turkey neck”. The MI Neck procedure involves sewing the muscle bands together and removal of extra skin to re-contour the neck area. Another contouring option is submental liposuction to “sculpt” the tissues so the neck looks like it did many years ago.

We have many options to improve your appearance. Schedule a consultation by calling 805-563-0000 to discuss if an MI procedure (versus more extensive surgical treatments) may be right for you.

Are You Bothered By Your Ears?

Many of our patients have felt a dissatisfaction with their ears from a very young age. The most common concern related to ear appearance is that they “stick out” too much. Otoplasty – often referred to as “ear pinning” surgery – can change the relationship of the ear to the face and the side of the head.

Here are a few facts about Otoplasty:

  • It can be performed on a child as young as 6 or 7 years old (unlike most other plastic surgeries) because at that age the ears are 90% of the size they will be as an adult.
  • This surgery is not just for ears that stick out.
  • It can address the shape and symmetry of the ears, including reducing the size of the earlobes if they are too large.
  • The surgery does not require general anesthesia. It can be performed under local anesthesia combined with oral sedation alone or together with an IV sedation.
  • Pain is usually minimal and easily controlled with Ibuprofen or Tylenol.
  • The recovery time for an adult or a child is about 1 to 2 weeks.

A successful Otoplasty can be a dramatic transformation for a child, a teen, or an adult.

Eyelid Lift Surgery in Santa Barbara

Eyes are often the first thing people notice. They convey so much without a word being spoken.

Unfortunately, aging, stress, smoking, excess drinking and/or sun damage can dramatically affect the delicate skin around the eye area resulting in sagging skin, fat bulges, hooded eyelids, fine lines, wrinkles, and/or bags. When any (or many) of these symptoms arise, a person may appear tired, unhappy or unapproachable when they are not.

Fortunately we can restore the appearance around the eyes with blepharoplasty – the medical term for eyelid enhancement surgery.

Here are your options for eyelid surgery in Santa Barbara.

Upper Eyelid Surgery addresses:

  • Excess or overhanging upper eyelid skin
  • Inner corner fat bulges

Facts About Upper Eyelid Surgery:

  • Incisions are made in the natural creases of the eye and are well hidden when healed.
  • A conservative approach is generally better to avoid looking “operated”. Especially for men, too much visible lid gives a feminine appearance.

Lower Eyelid Surgery addresses:

  • Dark circles
  • Fat bulges
  • Skin laxity under the eyes

Facts About Lower Eyelid Surgery:

  • For those who have fat bulges but little excess skin, incisions are made on the inside of the lower lid. Fat is removed and sutures are usually not required.

General Facts About Eyelid Surgery

  • General anesthesia is not need for eyelid surgery.
  • We can perform the surgery under local anesthesia combined with oral sedation alone or together with IV sedation.
  • Pain is usually minimal and easily controlled with Tylenol or Ibuprofen.
  • Most people take one week off work for recovery.

At the Cosmetic Surgery Center in Santa Barbara, our goal is to give you  natural looking rejuvenation so other people will notice that you look well-rested but will most likely not guess that you have had something done.

Neck Rejuvenation in Santa Barbara

Is your neck giving away your “true” age?

The neck area is comprised of delicate skin and as we age it becomes susceptible to wrinkles, pigmentation, and a “hanging” appearance. The reasons for this include sun damage, gravity, fat accumulation and heredity. Neck rejuvenation is a wonderful way to restore youthful contours and make a positive difference in your appearance. As the Cosmetic Surgery Center in Santa Barbara, we offer various surgical neck rejuvenation options (all done without general anesthesia) and a surprising non-invasive option for a younger-looking neck.

Neck Lift. If your neck has sagging skin with excess wrinkling, neck lift surgery is a good option.

  • The skin is tightened and muscles are often tied together in the midline to help create a “sling effect” for better support.
  • Excess fat is removed and a suspension suture is often placed to give more long-lasting support to the neck.
  • The surgery is usually done hiding incisions behind the ears and a small one under the chin.

Submentoplasty. The area under your chin is also known as the submentum and will often develop bands of excess and “stringy” muscles as we age. A submentoplasty is less extensive than a neck lift procedure:

  • The muscle bands are sewn together and extra fat is trimmed.
  • Sagging skin is removed to re-contour the neck.
  • A short zig-zag incision is hidden behind the chin.
  • Especially good for men who are starting to see a “turkey neck”.

Submental Liposuction. Improving your double chin can often be done with liposuction. This procedure will remove excess subcutaneous fat and is best done on patients with good skin tone.

  • Liposuction can address the submentum, anterior neck and even some of the jowls.
  • It can be done through a tiny incision hidden behind the chin.
  • When properly done it will “sculpt” the tissues so the youthful contour of your neck will be restored.

Platysma Plication. The platysma muscle stretches from the clavicle (collar bone) to the lower face. As we age the skin elasticity of the neck decreases and contraction of the platysma starts to cause banding of the overlying skin creating a “turkey neck” appearance.

  • Plication means sewing the right and left platysma muscles together in the midline to allow them to return to their normal position.
  • This is accomplished through a small incision hidden in the crease behind the chin.

Minimally-Invasive Neck Suture Lift. This surgery is designed to lift and support the neck skin with special sutures that encourage scar tissue in-growth to “attach” the skin to the suture.

  • Sometimes referred to as a “thread lift”.
  • Less invasive than a traditional neck lift with a shorter healing time.
  • Ideal for a person who’s aging neck issues are in the earlier stages.

Non-Surgical Neck Rejuvenation. Many people are surprised to find out that Botox and Dysport an be used for non-surgical neck rejuvenation.

  • Botox or Dysport is injected into the platysma muscle causing relaxation of the muscle.
  • When the muscle is relaxed, the appearance of the neck banding or “turkey neck” is reduced.
  • Results last about 3 to 4 months.

Neck rejuvenation can truly turn back the clock on your appearance and there are many options for different stages of the aging neck.
