Rhinoplasty – commonly referred to as a “Nose Job” – can be done to help with the appearance of a nose and can also address obstructive airway problems. A person’s nose is one of the most prominent features on the face – it should look good and function well.

Rhinoplasty can alter both the profile and frontal views. It can:

At Cosmetic Surgery Center, we perform the majority of nasal surgeries with a closed technique that hides the incisions inside the nose. The external shape can be corrected or changed by altering the bone, cartilage and soft tissue underneath the skin, allowing for a more ideal relationship among the parts.

Most people are relieved to hear that general anesthesia is not needed. A combination of local anesthesia, oral and intravenous sedation keeps the patient comfortable and unaware. After surgery pain in usually minimal and can easily be controlled with oral medications. An external dressing is in place for one week after surgery. Most patients return to work in 7 to 10 days and can resume exercise after 3 weeks. Swelling subsides within the first 2 months. Subtle refinement continues to appear as time passes and final results will be seen about 1 year after surgery.

Rhinoplasty is not usually performed until a person has reached the late teenage years when growth is nearly complete. It can be done in conjunction with a chin or cheek augmentation to improve overall balance.

Forehead Lift Surgery in Santa Barbara

When a person’s forehead has relaxed, causing the eyebrows to lower and the eyes to be crowded, they may appear angry, tired, or sad (when they are not). Many people think that it is their eyes that need to be done and are surprised to hear that it is the forehead area that needs to be addressed.

At Cosmetic Surgery Center in Santa Barbara, we offer Forehead lift surgery – also referred to as a “brow lift” – to reposition the forehead and brow upward. This one procedure gives multiple benefits:

  1. Raises the eyebrows to their normal position which opens the eyes and “removes” excess upper lid skin – try it yourself in the mirror.
  2. Smooths the horizontal forehead lines.
  3. Improves crow’s feet by tightening the temporal area.
  4. Removes/softens the frown lines between your eyebrows.

Forehead lift surgery is done through three small incisions in the hair (hair does not need to be shaved). This allows access to the muscles that cause frowning. Partial removal of these muscles causes permanent weakening of the muscle while retaining some function for normal expression. The forehead tissues are shifted upward and held in place with dissolvable sutures for the healing process.

As with all of our cosmetic surgeries, general anesthesia is not needed. Local anesthesia and oral sedation alone, or in combination with intravenous sedation, provides relaxation and amnesia. Pain is usually minimal and easily controlled with Tylenol or Ibuprofen. Most people return to work in 5 to 7 days after surgery.

We do not overdo the tissue lift because that can leave a person looking surprised and “operated”. Instead we take a more conservative approach so the result will be a natural, rested appearance.

Facelift Surgery in Santa Barbara by Dr. Perkins

Facelift surgery (cheek/neck lift) restores a youthful, natural appearance. When a person has a facelift, the results include:

  • Returning fullness to the cheeks
  • Lifting of the jowls
  • Smoothing the neck

What is done during Facelift Surgery?
Some surgeons simply pull the facial skin tighter which can result in an “operated” or “wind tunnel” appearance. Dr. Perkins feels it is important to lift and support the deep tissues so the skin tightening doesn’t need to be the main component of lift. By doing this patients should never have a stretched appearance and the deeper support will add longevity to the result. Incisions are hidden in the natural creases around the ears and in the hair (the hair does not need to be shaved).

Does Facelift Surgery Require General Anesthesia?
No, it does not. Many people are surprised to hear that general anesthesia is not needed for facelift surgery. Instead, we use a combination of oral, IV and local anesthesia to provide relaxation and control any discomfort. And with this technique patients usually have little or no pain, so Tylenol or ibuprofen is all that is needed during recovery. A light elastic dressing is used for the first 4-5 days and most patients return to work in 8-10 days.

Santa Barbara Cosmetic Surgery: Facial Augmentation with Implants

If a person feels that one of their facial features is “weak” and thereby creating an imbalance in their appearance then facial augmentation with implants may be a good choice for them. Facial implants are popular because they are a more permanent option for aesthetic improvement (as opposed to dermal fillers) and because they come in many shapes and sizes or can be customizable to individually suit the person’s face.

At our Santa Barbara Cosmetic Surgery Center, we can use facial implants for all areas of the face.

Cheek Implants add volume and contouring to the cheek area that may have lost the curvature due to genetics, age or extreme weight loss. When fat is lost in the cheek area, the hollows in the under eye area and the nasolabial folds may become more pronounced, making a person appear tired or unhappy. Cheek implants restore the volume and give the overall face a “lift”.

Chin Implants are used to strengthen a chin that has a noticeably weak definition. An under-projected chin can make the nose and/or neck appear larger than they are. The proper size and shaped chin implant will re-create definition. As an extra bonus, we often hear patients say the newly defined chin makes them look like they have lost weight!

Nasal Implants can be used to elevate the nasal bridge and/or improve the tip. The implant can be custom-sized to best fit the person’s face and compliment the other features.

Mandibular Angle Implants are used to create a stronger and more defined jaw. This surgery is more common for men because a wider lower face is seen as desirable.

Santa Barbara Cosmetic Surgeon Addresses Liposuction Myths

Liposuction was the 2nd most popular cosmetic surgical procedure in 2016 (breast augmentation was the 1st). Everyone has heard of liposuction but there are still myths that we get asked about at our Santa Barbara cosmetic surgery practice. Here we address some of the most common preconceptions:

Liposuction is a great procedure for weight loss. Not true! Liposuction is a body contouring procedure, not a weight loss procedure. Most of the patients we treat are near, or at, their ideal weight and want to get rid of stubborn areas of fat that are resistant to exercise and a healthy diet such as the abdomen, flanks, inner and outer thighs, or upper arms.

Liposuction is only for women. Again, not true. While most of the people seeking liposuction are women, men also have the procedure done. In 2016, 11% of liposuction procedures done were on men. Popular treatment areas include the “love handles”, abdomen, and neck for men.

Liposuction gets rid of cellulite. Liposuction removes deep fat just beneath the skin but it may not necessarily remove the cellulite. At my Santa Barbara medical spa, Evolutions, there is a one-time cellulite treatment called Cellfina that is clinically proven to treat the primary structural cause of cellulite and offers long term results (3+ years).

Abdominal liposuction and tummy tucks are the same thing. Liposuction and tummy tucks are not the same thing. A tummy tuck (abdominoplasty) is done to address excess abdominal skin and tightening of the abdominal muscles. Liposuction simply removes the fat. Liposuction is often used in a tummy tuck procedure.

Liposuction is much more expensive than CoolSculpting or SculpSure. This may not necessarily be true. The price difference may be much less than you think. Non-invasive body contouring procedures (such as CoolSculpting and SculpSure) take multiple treatments. CoolSculpting.com states that cost “typically ranges between $2,000 to $4,000” for a personalized treatment plan.

At the annual IMCAS Aesthetic Surgery & Cosmetic Dermatology World Congress it was reported that the cosmetic surgery market worldwide grew by 8.3% in 2016 with about $8.9 billion spent on cosmetic surgery procedures and non-invasive aesthetic treatments. Plastic Surgery Trends in 2015

Last week the American Society of Plastic Surgeons (ASPS) released plastic surgery statistics report for 2015. These statistics included surgical and minimally-invasive procedures such as laser skin treatments, Botox, and chemical peels.

In a press release by the ASPS, they stated that “Since 2000, overall procedures have risen 115 percent, but the types of procedures patients are choosing are changing.

Last year there were 1,706,106 plastic surgery procedures performed in the US (up 2% from 2014).

When is the Right Time to have Cosmetic Surgery?

“When is the right time to have cosmetic surgery?” As a plastic surgeon in Santa Barbara, I am often asked this question.

The fact is that the “right time” will be different for everyone. Every person ages at their own unique pace, so I do not like to say that the “right time” to have a facelift is between 45 and 55 because someone may be a candidate for facelift surgery at a younger age and others may be able to wait until their later years. Also, for most people, they must consider the cost of the surgery and recuperation time.

Solutions for An Aging Face

CSC_Jan_2016As we age, our body changes and our facial features tend to lose their youthful shape. The production of collagen and elastin slows and the skin’s ability to repair itself starts to slow down. Gravity also plays a roll in an aging face as fat starts to shift downward.

The good news is that nowadays there are multiple ways to treat facial aging. In this blog post I will touch upon a few of the most popular facial aging concerns that I see and discuss aesthetic treatments or plastic surgery procedures that can help turn back the clock.

SAGGING JOWLS
Problem: The ligaments along the jawline have weakened and the jowls are sagging.
Solution: A natural, youthful appearance can be restored with Facelift surgery by returning fullness to the cheeks, lifting the jowls and smoothing the neck.

LOSS OF VOLUME
Problem: Facial volume has decreased and settled, resulting in the lower face becoming “heavier” or more prominent.
Solution: The Layered Liquid lift is a non-surgical solution for volume loss. A variety of injectable fillers (such as Radiesse and Restylane) are done in a “layering” technique to plump the skin from the inside out. Wrinkles are filled and fullness is restored.

DEFLATED CHEEKS
Problem: Cheeks have lost their volume and appear flat which lengthens the face.
Solution: Deflated cheeks can be treated with a minimally-invasive Cheek Lift with fat grafting. A cheek lift elevates the fat pad in the cheek to re-establish fullness to the eyes and cheeks. A small amount of fat is harvested (usually from the abdomen or thighs) to add additional volume where it may be needed, such as in the nasolabial lines (the lines from the side of the nose to the corners of the mouth) or the “comma” lines around the mouth. A Cheek Lift can also be done without fat grafting.

WRINKLES AND UNEVEN PIGMENTATION
Problem: Wrinkles and uneven pigmentation, usually from sun damage.
Solution: Chemabrasion involves a medium-depth chemical peel with a light dermabrasion (not microdermabrasion) which usually gives a person 70 to 80% improvement in the appearance.

Over my 30 years of cosmetic surgery practice, I have learned that there is not a one-size-fits-all solution for aging concerns. While the solutions above may be ideal for some, others may benefit from a different cosmetic surgery or aesthetic treatment.
