FAQs About Cosmetic Surgery in Santa Barbara

Q: I am not sure if I am ready for surgery, but I am interested. Should I come in now for a consultation?
A: We recommend coming in for a consultation when you have specific concerns. We will evaluate your concerns and together come up with a plan.

Q: How long do I have to wait from consultation to the day of surgery?
A: Generally your surgery can be scheduled within 2 to 4 weeks after your consultation.

Q: Where are the surgeries performed?
A: We perform all of our surgeries in our outpatient facility that is located near Cottage Hospital.

Q: Do you take medical insurance? Will you bill my insurance company?
A: No. We do not take medical insurance and we will not bill your insurance company.

Q: How much will my cosmetic surgery cost?
A: Cost depends on the procedure(s) performed. A written quotation will be given to you at the time of your consultation.

Q: Do you have payment plans?
A: Yes, you can set up a payment plan through Care Credit. Read more about Care Credit here.

Q: Is my medical information confidential?
A: Your medical information is always kept private. It can be discussed with your family member only if you say OK.

Q: Do I have to go under general anesthesia?
A: No. All of our facial surgeries and body liposuction can be performed safely and comfortably using local anesthesia and conscious sedation.

Q: How much pain can I expect after surgery?
A: Most facial procedures (including full face lifts) and body contouring surgeries have remarkably little pain – and can be easily controlled with oral medication.

Q: I have some medical issues – can I still have surgery?
A: It will depend on what your medical issues are. As cosmetic surgeries are usually elective, there is never a reason to put any patient at risk. If necessary, “medical clearance” will be requested from your family MD.

Q: I’m from out of town. Where do you recommend that I stay?
A: We have a webpage for visitors to Santa Barbara including hotel recommendations, dining options, and attractions that you may be interested in during your visit.

Q: I live locally, can I drive myself home?
A: No. Please make arrangements for a friend or relative to take you home after the surgery.

Plastic Surgery Statistics for 2018

This month the annual 2018 Plastic Surgery Statistics Report was released by the American Society of Plastic Surgeons (ASPS). Cosmetic surgery procedures overall went up by 2% with 17.7 million procedures performed.

The top 10 cosmetic surgery procedures in 2018 were:

  1. Breast Augmentation: 313,735
  2. Liposuction: 258,558
  3. Rhinoplasty: 213,780
  4. Eyelid Surgery: 206,529
  5. Tummy Tuck (Abdominoplasty): 130,081
  6. Facelift: 121,531
  7. Breast Lift: 109,638
  8. Dermabrasion (not to be confused with a day spa Microdermabrasion treatment): 80,697
  9. Neck Lift: 53,571
  10. Forehead Lift: 38,795

Seven of the top 10 treatments focused on the neck or above. Trends in plastic surgery have been leaning towards results that look natural and not “done”.

Minimally Invasive Cosmetic Surgery in Your 40s

Cosmetic surgery is becoming more prevalent among 40-somethings than ever before. The upward trend amongst this age group may stem from our culture’s obsession of staying “forever young” coupled with the pressure to look good in social media pictures. But most importantly, the 40-somethings that we see at our Santa Barbara Cosmetic Surgery Center simply say that they don’t “feel in their 40s” even though their face may say otherwise. 

Aging is always happening, but the decade of the 40s is usually the time when visible signs start to appear. Collagen and elastin production in the skin slows down while sun and environmental damage start to show up, and the result is fine lines, wrinkles, age spots, skin laxity and sagging. 

While there are many “non-invasive” options such as Botox / Dysport, dermal fillers, and lasers, more 40-somethings are turning toward more permanent options such as cosmetic surgery because they aren’t interested in multiple med spa visits to keep up with Botox or fillers and/or they are no longer getting the optimum results from those treatments.

The good news is that for most people in their 40th decade, there are more options than if they waited two more decades to rewind the clock. The essential foundation of the face is still present in the 40s and often minimally invasive (MI) cosmetic surgery procedures can be used to achieve the desired result. (We must add that MI procedures can be a good option for those who want to “maintain” the benefits of a previous plastic surgery.)

MI cosmetic procedures give noticeable improvement, cost less, and have less recovery “downtime”. In addition, they have smaller or no visible incisions. MI treatments can address all areas of the face – the forehead, eyebrows and temporal region, lower eyelids, cheeks and mid-face, and the area under the chin (submentum) – and the hands.

Cosmetic Procedures and Plastic Surgery Trends in 2019

Cosmetic procedures and plastic surgery will continue to rise in 2019, however, the days of getting over-exaggerated plastic surgery (overfilled lips, disproportionate breasts) are on their way out. The future of plastic surgery and non-invasive cosmetic procedures (i.e. dermal fillers, Botox, lasers) is trending towards enhancing what a person already has and making slight tweaks so they look more refined “naturally” while still staying true to their inherited features.

Here is what we expect to see in 2019 on the cosmetic and plastic surgery front.

Minimally-Invasive Treatments for Preventative Measures or Refinement 

More people will seek Minimally-Invasive (MI) cosmetic surgery including those who want to delay a larger plastic surgery or those who want to “maintain” the benefit of a previous surgery. MI surgery is ideal because it can give a noticeable improvement with less cost and less “downtime”.  In fact, one MI procedure can often be much less of an expense than multiple medical spa treatments. MI surgery can treat many facial areas, the area under the chin, and hand/body contour defects. Here is a brief description of MI procedures that we do:

  • A Temporal Lift elevates the outer half of the brow and improves crow’s feet.
  • An MI Cheek Lift tightens tissue with a small amount of skin excision to replace lost volume to the cheek area. Volume can also be added to the mid-face, nasolabial folds, and pre-jowel area through injections of your own fat (taken from a place where you want to have less of a fat bulge).
  • Lower Eyelids can be treated by removing a small “pinch” of excess skin with fat removal from inside the lower lid (so there are no sutures or external scars).
  • Add Facial Volume via fat grafting. Fat can be places in the brows, temporal hollows, under the frown lines, deep circles around the eyes, apples of the cheeks, cheekbone region, groove between the nose and the corner of the mouth, lips, pre-jowel area (marionette lines), and the jawline.
  • Hand Rejuvenation can be done by fat transplantation to replace volume loss in the back of the hands. Additionally pigment spots can be treated with lasers.
  • A Submentoplasty is less extensive than a neck lift and is especially good for men who are starting to see a “turkey neck”.

Injectable Cosmetic Procedures to Stay on the Rise

Injectables (Botox, Dysport, or dermal fillers like Restylane) will stay on the rise because of the accessibility and the affordability of these treatments. They are also appealing because most results are instantaneous with little to no downtime. A dermal filler procedure can easily be done on an extended lunch hour. We are also seeing a younger audience turn to injectables for enhancements to help them look their best in their close-up “selfie” photos they post on social media.

Body Contouring Will Continue to Be Popular 

There are many non-invasive body contouring procedures that have been introduced over the past few years including freezing fat or melting it off for fat reduction in a targeted area. People are drawn to these treatments because they do not involve surgery and have less of a downtime for recovery. Most of these treatments claim up to 20% reduction – this is optimal and may take more than 1 treatment to achieve. They may improve contours but results may be minimal and not noticeable to other people. Multiple treatments may actually cost more than one liposuction treatment for the same area. At the Cosmetic Surgery Center, we offer liposuction to improve body contours and proportion. Liposuction results are guaranteed and skin excision can take place at the same time, if necessary. People no longer need to go under general anesthesia for liposuction as it can be done comfortably using a combination of local anesthesia, oral and intravenous sedation. Are You Bothered By Your Ears?

Many of our patients have felt a dissatisfaction with their ears from a very young age. The most common concern related to ear appearance is that they “stick out” too much. Otoplasty – often referred to as “ear pinning” surgery – can change the relationship of the ear to the face and the side of the head.

Here are a few facts about Otoplasty:

  • It can be performed on a child as young as 6 or 7 years old (unlike most other plastic surgeries) because at that age the ears are 90% of the size they will be as an adult.
  • This surgery is not just for ears that stick out.
  • It can address the shape and symmetry of the ears, including reducing the size of the earlobes if they are too large.
  • The surgery does not require general anesthesia. It can be performed under local anesthesia combined with oral sedation alone or together with an IV sedation.
  • Pain is usually minimal and easily controlled with Ibuprofen or Tylenol.
  • The recovery time for an adult or a child is about 1 to 2 weeks.

Rhinoplasty in Santa Barbara at Cosmetic Surgery Center

Rhinoplasty – commonly referred to as a “Nose Job” – can be done to help with the appearance of a nose and can also address obstructive airway problems. A person’s nose is one of the most prominent features on the face – it should look good and function well.

Rhinoplasty can alter both the profile and frontal views. It can:

  • Refine a bulbous tip
  • Narrow a wide nose
  • Straighten a crooked nose
  • And/or remove an unwanted bump

Along with helping the appearance, nasal surgery can also address function (such as a deviated septum or bone spurs) if needed.

At Cosmetic Surgery Center, we perform the majority of nasal surgeries with a closed technique that hides the incisions inside the nose. The external shape can be corrected or changed by altering the bone, cartilage and soft tissue underneath the skin, allowing for a more ideal relationship among the parts.

Most people are relieved to hear that general anesthesia is not needed. A combination of local anesthesia, oral and intravenous sedation keeps the patient comfortable and unaware. After surgery pain in usually minimal and can easily be controlled with oral medications. An external dressing is in place for one week after surgery. Most patients return to work in 7 to 10 days and can resume exercise after 3 weeks. Swelling subsides within the first 2 months. Subtle refinement continues to appear as time passes and final results will be seen about 1 year after surgery.

Rhinoplasty is not usually performed until a person has reached the late teenage years when growth is nearly complete. It can be done in conjunction with a chin or cheek augmentation to improve overall balance.

Forehead Lift Surgery in Santa Barbara

When a person’s forehead has relaxed, causing the eyebrows to lower and the eyes to be crowded, they may appear angry, tired, or sad (when they are not). Many people think that it is their eyes that need to be done and are surprised to hear that it is the forehead area that needs to be addressed.

At Cosmetic Surgery Center in Santa Barbara, we offer Forehead lift surgery – also referred to as a “brow lift” – to reposition the forehead and brow upward. This one procedure gives multiple benefits:

  1. Raises the eyebrows to their normal position which opens the eyes and “removes” excess upper lid skin – try it yourself in the mirror.
  2. Smooths the horizontal forehead lines.
  3. Improves crow’s feet by tightening the temporal area.
  4. Removes/softens the frown lines between your eyebrows.

Forehead lift surgery is done through three small incisions in the hair (hair does not need to be shaved). This allows access to the muscles that cause frowning. Partial removal of these muscles causes permanent weakening of the muscle while retaining some function for normal expression. The forehead tissues are shifted upward and held in place with dissolvable sutures for the healing process.

As with all of our cosmetic surgeries, general anesthesia is not needed. Local anesthesia and oral sedation alone, or in combination with intravenous sedation, provides relaxation and amnesia. Pain is usually minimal and easily controlled with Tylenol or Ibuprofen. Most people return to work in 5 to 7 days after surgery.

We do not overdo the tissue lift because that can leave a person looking surprised and “operated”. Instead we take a more conservative approach so the result will be a natural, rested appearance.

Facelift Surgery in Santa Barbara by Dr. Perkins

Facelift surgery (cheek/neck lift) restores a youthful, natural appearance. When a person has a facelift, the results include:

  • Returning fullness to the cheeks
  • Lifting of the jowls
  • Smoothing the neck

Mid-facial implants or fat grafting might be suggested to further enhance the procedure if indicated.

What is done during Facelift Surgery?
Some surgeons simply pull the facial skin tighter which can result in an “operated” or “wind tunnel” appearance. Dr. Perkins feels it is important to lift and support the deep tissues so the skin tightening doesn’t need to be the main component of lift. By doing this patients should never have a stretched appearance and the deeper support will add longevity to the result. Incisions are hidden in the natural creases around the ears and in the hair (the hair does not need to be shaved).

Does Facelift Surgery Require General Anesthesia?
No, it does not. Many people are surprised to hear that general anesthesia is not needed for facelift surgery. Instead, we use a combination of oral, IV and local anesthesia to provide relaxation and control any discomfort. And with this technique patients usually have little or no pain, so Tylenol or ibuprofen is all that is needed during recovery. A light elastic dressing is used for the first 4-5 days and most patients return to work in 8-10 days.

Arm Liposuction in Santa Barbara

Saggy upper arms can be caused by decreased collagen and elastin levels (which happens naturally as we age) or from weight gain. When the upper arms are flabby, they may have a “batwing” or “kimono” appearance that may make someone self conscious. Unfortunately, the localized fat and loose skin of the upper arm can be one of the most stubborn areas to shed or tone, even with proper diet and exercise. When this is the case, arm liposuction can make a big difference in the appearance of the arm.

Arm liposuction can be done with or without skin excision. When fat is the main issue, it can usually be treated with liposuction alone and the skin will contract without the need for skin removal. When there is too much excess skin, commonly this happens after weight loss, skin removal can be done along with the liposuction. The skin removal scar is placed on the inside of the arm (against the body), above the elbow, and is not visible from the front or back. If only a small amount of skin is removed, the skin removal can often be confined to or near the armpit.
