Patient Resources

Your First Appointment:  Please plan to arrive 15 minutes early for your first scheduled appointment so you have time to fill out new patient paperwork.  After your consultation, follow-up appointments will be set up that is convenient and that still allow us to address your concerns in a timely manner.

Cancellation Policy:  If you should need to cancel or reschedule an appointment, we ask that you please give us at least 24 hours’ notice.  Thank you for this consideration.

Payment Policy:  Payment is usually due on the day of the treatment for injectables or minor procedures not done in the operating room.  For surgical procedures scheduled in the O.R., payment is due on or before the pre-operative visit.  We accept cash, personal checks (with ID), MasterCard, Visa and Care Credit.

Parking:  Parking is available on the Cottage Hospital side of the building, with access off of Nogales Street.
